If you are a new patient, you will need to complete a medical records release form during your visit. Upon completion of the form our staff will facilitate the transfer of your documents.

Your healthcare provider may request your medical records from our office at anytime. If you are a Torrance office location, please contact our main office line at 310-378-2234 and for Manhattan Beach office patients, please call (310) 939-7858

How do I get a copy of my medical records for myself?

You may request a copy of your medical records (including X-rays, test results, etc.) at any time for your personal records. Contact our office for more information.
