  • What insurances do you accept?
    • We accept most major PPO and Medicare insurance plans, no HMO contracts. For more information please visit our insurance guide page.

Paper Work & Medical Records

  • How do I transfer my medical records?
    • In order to have your medical records transferred to SBFMG (South Bay Family Medical Group), you may choose from the following options:
      • You may call your previous medical provider and request they send us your medical records by official postage mail.
      • Fill out a medical release form at our office and SBFMG staff will work on obtaining your records from your previous provider.
  • How do I get a copy of my medical records for myself?
    • In order to obtain a copy of your medical records you must submit a written request or fill out a medical records request form, located in our office.
  • How do I refill my prescription?
    • Call your pharmacy and ask them to fax a request to your doctor at SBFMG (South Bay Family Medical Group
  • Is there a charge to complete paperwork?
    • Completing patient requested forms requires office staff time and time away from patient care for our doctors. Due to the time and expense incurred in reviewing patient files and filling out forms, certain fees will be charged. However, fees may be waived if forms are completed in conjuction with a scheduled appointment with your physician. Click here to see a list forms and their related fee.

Test Results

  • How do I get a copy of my lab results?
    • In order to obtain a copy of your lab results, you may choose from the following options:
      • You may call our office and request for a copy to be mailed to you
      • A may request that a copy be faxed to you
      • Pick up a copy of your lab results upon your next appointment
  • Where can I have my lab work drawn?
    • At SBFMG, we offer a fully accredited and licensed diagnostic laboratory is able to process a broad spectrum of laboratory testing. With our group of highly skilled medical professionals you can trust that your lab specimens will be tested quickly and accurately, with rapid turnaround of results.   • How long does it take to get my lab results?
    • Most test done in our in-house laboratory your results will be available within 24 hours of your testing. However, during special circumstances your doctor may order from an outside reference lab. In the event that this occurs, the results may take between 24 hours to 14 days, depending on the individual lab.
  • Can you email my results to my email address?
    • At this time standard email is not HIPPA complaint. We are in the process of implementing a secure HIPPA complaint lab result, coming 2012.

  • Do you accept walk-in appointments?
    • We do not offer walk-ins. However, we do offer same day appointments. Please call our office for more information.
  • Do you have urgent care hours?
    • South Bay Family Medical Group has offered urgent care hours for nearly thirty years. The purpose of Urgent Care is to provide convenient services for non-emergency related medical problems that develop after office hours on Saturdays or holidays when the medical offices are traditionally closed. Urgent Care Centers Open Saturday 9:00 a.m. - 12:00 p.m.

  • Can you provide me with directions?

      For Directions from the 405 South
      Exit Hawthorne Blvd and turn right
      Go about 4.5 miles and turn left on Skypark Dr
      Our office will be on your left side
    • For Directions from the 405 North
      Go on the Harbor Fwy 110 South
      Exit Sepulveda and turn right
      2 miles and turn left on Crenshaw Blvd.
      1 mile and turn right on Skypark Drive
      Go about 1.5 miles, our office will be on your right side

      Exit Sepulveda Blvd. and turn right

  • Where can I drop off my biohazard containers and expired medication?
    • Properly disposing of your used sharps and unwanted or expired medication will help protect yourself, your family, community, the public and environment. Click here to see a list of dispensing locations.
  • Is parking available for free?
    • Yes
  • What do I do in case of an emergency?
    • If it is a life threatening emergency please call 911.
  • What if it is an urgent, non-emergency matter?
    • We have an on-call physician that is available after hours. Please call our main line and follow the prompt.
