Appointment Checklist

First Appointment

Please arrive at least 30 minutes prior to your first appointment. The following should be brought with you to your first appointment:

1. Completed New Patient Forms

Please have the above forms filled and printed out prior to your visit. If forms are not filled out prior to visit, staff will have you fill them out when you arrive.

2. Current insurance card(s)

Please have a copy of your current Insurance card(s) for verification by our staff.

3. Current Rx Card

If you have a pharmaceutical Rx card that was provided to you by your insurance please bring it to your visit.

4. Valid photo ID

For your protection, federal regulations require that patients provide photo identification when presenting for appointments.

5. Medicines that you are currently taking

Please bring the medicines in their actual bottles so your medical care provider can know exactly what medications and dosages you are on.

6. Medical Records

If you are a new patient, you will need to complete a medical records release form after your initial visit. Our office will handle the transferring of records.

7. Bring Your Co-Pay

In order to process you as a patient we must receive a co-pay from you first.

Future Appointments
For future appointment, please arrive at least 15 minutes prior to your appointment. The following should be brought with you to every appointment:

  • • Current insurance card(s)
  • • Current Rx/Prescription Cards
  • • Valid photo ID
    • A list of all you current medications, including prescription and over the counter medications
    • A list of all known allergies

Please be prepared to bring applicable insurance copay, deductible or payment in full if you are not covered by insurance. Payment depends upon your insurance type and plan. Payments are collected when you check in.
